Abstract: This article is part of an independent research on the reform of the operational aspects of the District Health Promotion Hospital in Phayao province. The study explores the changed working context of the District Health Promotion Hospital in Phayao after the transfer of responsibilities to the provincial level. The analysis in this article is based on data collected from questionnaires between December 2023 and January 2024, from the personnel of community health promotion hospitals, the Provincial Administrative Organization of Phayao, to explain changes in personnel administration, budget and procurement management, and organizational culture. The researcher conducted the study using a questionnaire survey and focus group discussion. The significant findings from this study focus on the delegation of tasks from the Tambon Health Promotion Hospital. This has led to the development of operational processes in various dimensions that are more convenient and efficient for the hospital's personnel. Additionally, it has increased opportunities for improvement and innovation to enhance the value of health services at the local and community levels in Phayao province more effectively and with higher quality. This includes the creation of collaboration and teamwork among relevant organizations at the provincial and community levels to support the delivery of health services that are beneficial to the people in Phayao appropriately.
