Abstract: Nowadays, cancer is increasing every year in many countries as a result, the waiting time for treatment increases. This research aims to apply a mathematical model for radiotherapy planning and scheduling problems with capacity constraints over a multi-period time. The goal is to schedule a list of 3D Curative Care patients who need to get radiation treatment. The problem is formulated with a multi-objective model with two stages: minimizing the makespan (Z_1) and priority patient score (Z_2) in the first stage. minimizing the sum of the finish time (Z_3), the sum of different balance workloads (Z_4) and assigned patients to RT room score (Z_5) in the second stage. Mathematical models were developed using the simulated data according to real treatments of 3D Curative Care patients with sample data from 6 major cancers. All objectives are formulated and weight-normalization method. The results show that the model satisfies all constraints with the right assignment and operation sequence. The patient's waiting time from the previously fixed 28 days was reduced. Then compare the time results, the proposed mathematical model reduces planning time from hours to minutes. The weight sensitivity analysis in the objective equation, it was found that the linear relationship and showed no difference from the weighting change in stage 1 and the objective equations Z_3 between Z_5 in stage 2 was trade off relationship. This mathematical model can assist the decision-maker to planning cancer patient scheduling for radiotherapy treatment. The research is still ongoing to find ways to improve the effectiveness of the model for a wide variety of patient scheduling problems and larger problems
