Abstract: Patient safety is an important fundamental principle of health care services and is of concern to healthcare organizations and patients around the world. This descriptive study aimed to study patients' perceptions of safety and recommendations for improving patient safety at Maharaj Nakorn Chiang Mai Hospital. The participants consisted of 434 patients admitted to inpatient units at Mabaraj Nakorn Chiang Mai Hospital in Chiang Mai, Thailand. The research instruments were the Patient Measure of Safety questionnaire developed by Louch et al. (2019) and the Patient Safety Recommendation questionnaire developed by the researcher. The reliability of the Patient Measure of Safety questionnaire was 0.83 The data were analyzed using descriptive statistics and categorization. The results were as follows: 1. The patients in Maharaj Nakorn Chiang Mai Hospital perceived the overall patient safety at a high level. Domains that patients perceived at a high level were dignity and respect, staff training, communication and teamwork, information flow, organization and care planning, delays, access to resources and ward type and layout. The domain of staff roles and responsibilities was perceived at a moderate level. 2. The recommendations for improving patient safety in Maharaj Nakorn Chiang Mai Hospital were 1) gentle communication/action and respect, 2) provide general and specific care training, 3) improve communication for patients and families and between team or staff, 4) rapidly and systematically transfer patient information and improve the patient database for transferring patient information, 5) managing care for patient safety, 6) manage the number of personnel appropriately for the workload, increase and distribute the service places, and organize an effective work system, 7) manage the amount of well-maintained equipment so that it is ready to use, 8) improve the use of space and equipment in the unit for safety, control for appropriate light, noise, temperature, and cleanliness, and 9) take full responsibility. The results of this study can be used by the hospital and nurse administrators as guidance formanaging work systems and support resources to improve patient safety at Maharaj Nakorn Chiang Mai Hospital.
