การรักษาภาวะหัวใจห้องล่างเต้นผิดจังหวะจากภาวะท้องเสียเฉียบพลันในสุนัขพันธ์ลาบราดอร์รีทรีฟเวอร

Abstract

สุนัขพันธุ์ลาบราดอร์รีทรีฟเวอร์อายุ 9 ปีมีอาการช็อคจากการถ่ายเหลวเป็นจำนวนมากภายหลังพบภาวะหัวใจห้องล่างเต้นผิดจังหวะชนิด paroxysmal ventricular tachycardia จากเครื่องวัดคลื่นไฟฟ้าหัวใจรักษาเบื้องต้นด้วย lidocaine 2 มก./กก. และ 25 ไมโครกรัม/กก./นาทีจากนั้นให้ atenolol 0.2-0.5 มก./กก. ทุก 12 ชั่วโมงโดยการกินเป็นเวลา 3 วันพบว่าไม่สามารถคุมภาวะหัวใจเต้นผิดจังหวะได้จึงเปลี่ยนมาใช้ amiodarone 10 มก./กก. ทุก 12 ชม. จึงสามารถควบคุมการเต้นของหัวใจให้อยู่ในเกณฑ์ปกติโดยพบผลข้างเคียงระหว่างการใช้ยาคือค่าเอนไซม์ตับชนิด alanine aminotransferase (ALT) สูงกว่าค่ามาตรฐานเล็กน้อย

A 9 years old female Labrador retriever was shocked from severe acute diarrhea and paroxysmal ventricular tachycardia was observed from an electrocardiography. An initial treatment was performed with 2 mg/kg lidocaine bolus injection and continued with 25 microgram/kg/minute constant rate infusion. Atenolol was prescribed later but cannot control the arrhythmia despite increasing the dosages from 0.2 to 0.5 mg/kg per oral every 12 hours in 3 days. Replacing with amiodarone 10 mg/kg per oral every 12 hour was found to achieve in reversing the arrhythmia to normal sinus rhythm with a slightly increased of the alanine aminotransferase (ALT) enzyme during the course of treatment.
