การรักษาภาวะความดันหลอดเลือดแดงปอดสูงแบบไม่รู้สาเหตุในแมว ด้วยยา Baraprost Sodium

Abstract

แมวไทยพื้นเมืองขนสั้นเพศผู้อายุ 11 เดือนเข้ามารับการรักษาณโรงพยาบาลสัตว์มหาวิทยาลัยเกษตรศาสตร์บางเขนตั้งแต่เดือนมิถุนายน 2550 ด้วยอาการเหนื่อยง่ายเป็นลมและบางครั้งแสดงอาการอ้าปากหายใจเจ้าของสัตว์พบอาการดังกล่าวตั้งแต่อายุประมาณ 4 เดือนและได้รับการรักษาจากคลินิกเอกชนใกล้บ้านมาอย่างต่อเนื่องแต่อาการไม่ดีขึ้นผลการตรวจทางคลินิกแมวแสดงอาการอ้าปากหายใจและบางครั้งใช้ช่องท้องร่วมในการหายใจเมื่อมีการบังคับสัตว์ค่าโลหิตวิทยาอยู่ในเกณฑ์ปกติภาพถ่ายรังสีช่องอกพบลักษณะของหัวใจและปอดเป็นปกติภาพคลื่นไฟฟ้าหัวใจเป็นแบบ normal sinus rhythm และไม่พบการเบี่ยงเบนของแกนหัวใจแต่อย่างใดผลการตรวจหัวใจด้วยคลื่นเสียงความถี่สูงพบหัวใจห้องบนขวาและหัวใจห้องล่างขวาขยายใหญ่ ซึ่งเป็นลักษณะสัมพันธ์กับภาวะความดันหลอดเลือดแดงปอดสูงทําการรักษาด้วยการให้กินยา beraprost sodium ทุก 48 ชั่วโมงเพียงอย่างเดียวและทําการติดตามผลการรักษาพบว่าการหายใจและอาการเหนื่อยง่ายของแมวเริ่มดีขึ้นในสัปดาห์ที่ 2 ภายหลังจากการได้รับยาผลการตรวจหัวใจด้วยคลื่นเสียงความถี่สูงภายหลังจากได้รับยา 2 เดือนพบว่าหัวใจห้องล่างซ้ายกลับมามีขนาดเป็นปกติเนื่องจากไม่ถูกบีบกดโดยหัวใจห้องล่างขวาแมวที่ได้รับการรักษาภาวะความดันหลอดเลือดแดงปอดสูงแบบไม่รู้สาเหตุด้วยยา beraprost sodium ให้ผลตอบสนองต่อการรักษาเป็นที่น่าพอใจ

An 11 months old, Thai domestic short hair male cat was brought to Kasetsart University Veterinary Teaching Hospital, Bang Khaen in June 2007. The cat showed signs of exercise intolerance, syncope, and sometimes had opened-mouth breathing. The owner noticed these symptoms since he was 4 months old and had been treated previously by local veterinarian since then, but the symptoms remained unimproved. The cat had opened-mouth breathing and sometimes abdominal breathing after struggling with the restraint procedures. Hematological results were normal. The thoracic radiograph showed normal heart and lung patterns. ECG showed normal sinus rhythm with no evidence of heart axis deviation. The enlargement of right atrium and ventricle were revealed by echocardiogram which related to pulmonary arterial hypertension. The cat was treated with oral beraprost sodium every 48 hours. The clinical sign and clinical complain on exercise intolerance were improved after 2 weeks of treatment. Two months later, the echocardiography showed that the size of left ventricle return to normal because it had no abnormal pressure from the right ventricle. The treatment of cat with idiopathic pulmonary arterial hypertension using beraprost sodium showed an appropriate response.
