| Abstract: | วัตถุประสงค์ของการศึกษานี้ เพื่อเปรียบเทียบประสิทธิผลการบริหารยา tranexamic acid เพื่อลดการสูญ เสียเลือด และการให้สารเลือดทดแทนผู้ป่วยที่ผ่าตัดกระดูกสันหลังตีบแคบบริเวณเอวชนิด degenerative stenosis ในผู้ป่วยจํานวน 100 ราย โดยแบ่งผู้ป่วยออกเป็น 2 กลุ่มเท่า ๆ กัน และให้ tranexamic acid 10 มก./กก. ครั้งเดียวก่อนผ่าตัดเปรียบเทียบกับกลุ่มที่ให้ tranexamic acid 10 มก./กก. ก่อนผ่าตัด และให้ต่อ อีก 10 มก./กก. หลังผ่าตัด 24 ชั่วโมง เก็บรวบรวมข้อมูลปริมาณการสูญเสียเลือดระหว่างการผ่าตัดและหลัง การผ่าตัด ผลรวมของการเสียเลือดทั้งหมด พร้อมทั้งเจาะเลือดส่งตรวจทางห้องปฏิบัติ (hematocrit, hemoglobin) เพื่อดูความเข้มข้นของเลือดทั้งก่อนและการหลังผ่าตัด และบันทึกข้อมูลปริมาณการให้เลือด ซึ่งผล การศึกษาพบว่าการสูญเสียเลือดหลังการผ่าตัดลดลงอย่างมีนัยสําคัญทางสถิติ (p <0.05) ในกลุ่มที่ฉีดก่อน ผ่าตัดและให้ต่ออีก 24 ชั่วโมง เทียบกับกลุ่มที่ฉีดครั้งเดียว 717.80±313.75 มล. กับ 606.70±209.19 มล. และกลุ่มที่ฉีดก่อนผ่าตัดและให้ต่ออีก 24 ชั่วโมง ได้รับเลือดเพียง 10 ราย ในขณะที่กลุ่มที่ฉีดครั้งเดียวต้องให้ เลือดถึง 16 ราย จํานวนการสูญเสียเลือดระหว่างการผ่าตัด และการสูญเสียเลือดทั้งหมด ไม่แตกต่างกันอย่าง มีนัยสําคัญทางสถิติ รวมถึงค่าความเข้มข้นของเลือด (Hb, Hct) ซึ่งก็ไม่แตกต่างกันอย่างมีนัยสําคัญทางสถิติ |
