Abstract: The M.D. Anderson Dysphagia Inventory (MDADI) is a dysphagia-specific quality of life instrument to evaluate the impact of dysphagia on the quality of life in patients with head and neck cancer. The MDADI has been translated by a cross-cultural adaptation process and validated in many languages. However, there is no dysphagia-specific quality of life standardized instrument to measure the quality of life in patients with head and neck cancer in Thailand. The purpose of this study is to translated and validated The MDADI into Thai language by a cross-cultural adaptation process and determine the reliability of the Thai version of the M.D. Anderson Dysphagia Inventory (MDADI- TH). Additionally, a correlation between dysphagia severity and the quality of life in patients with head and neck cancer was verified. The original MDADI questionnaires were translated into Thai and validated by 6 steps of the cross-cultural adaptation process including forward translation, translation synthesis, backward translation, expert committee review, pretesting, and final proof reading. The reliability was determined by internal consistency and test-retest reliability. A total of twenty-nine head and neck cancer patients were able to complete the same translated MDADI questionnaires in two weeks after first enrollment. The MDADI-TH subscale and total scores showed acceptable internal consistency reliability (ranged from 0.71- 0.94). Similarly, Test-retest reliability was statistically significant (p < 0.05). For a correlation between the quality of life and dysphagia severity, a total of sixty patients were able to examined swallowing ability by VFSS. Therefore, the patients were included in an analysis of the correlation study. Correlation between the dysphagia severity and the quality of life was significant in total scores, emotional subscale, and function subscale (p < 0.05). In conclusion, The Thai version of the MDADI was showed good reliability. Therefore, the MDADI- TH can be a dysphagia-specific quality of life standardized instrument to measure the quality of life in Thai patients with head and neck cancer. These patients with more severity of dysphagia showed compromising on the quality of life, especially in emotional and functional domains.
