dc.description.abstract | This paper aims to introduce health management platform based-on seamless Electronic Medical Record (sEMR) approach. The purpose of our system is to help emergency patients who need urgent care and continuous care after being discharged from the hospital or referal case and apply the modern technology to better serve the limited humans. In the research methodology, the idea of sEMR data center framework is adopted, and the system is designed for exchange emergency patients data between the community hospital and the health promoting hospital. These also test the functions, reliability, and stability of the designed sEMR system. Moreover, the experiment results represent the effectiveness and the feasibility of the system. It can be proved that the seamless electronic medical record system is created for emergency patients who need urgent care and continuous care. In additional, it can provide greater contributions to manage health data of both patients and hospitals in the future. | en_US |
