ผลของการสอนโดยใช้เทคนิคไม่ใช้มือจับต้องต่อความสำเร็จในการเลี้ยงบุตรด้วยนมมารดาอย่างเดียวของมารดาที่มีบุตรคนแรก

Abstract

การเลี้ยงบุตรด้วยนมมารดาอย่างเดียวมีประโยชน์มากต่อมารดาและทารก รวมถึงส่งผลดีต่อครอบครัว สังคมและประเทศชาติ ปัจจัยสำคัญอย่างหนึ่งที่สามารถเพิ่มความสำเร็จในการเลี้ยงบุตรด้วยนมมารดาอย่างเดียวคือ เทคนิคการให้นมบุตรของมารดา การวิจัยครั้งนี้เป็นการวิจัยกึ่งทดลอง มีวัตถุประสงค์เพื่อศึกษาผลของการสอนโดยใช้เทคนิคไม่ใช้มือจับต้องต่อความสำเร็จในการเลี้ยงบุตรด้วยนมมารดาอย่างเดียวของมารดาที่มีบุตรคนแรก กลุ่มตัวอย่างคือ มารดาที่มีบุตรคนแรกจำนวน 52 รายที่นอนพักฟื้นภายหลังคลอดในหอผู้ป่วยสูติ-นรีเวชกรรม และตรวจหลังคลอดที่โรงพยาบาลตติยภูมิจังหวัดเชียงใหม่ ระหว่างเดือนพฤษภาคมถึงเดือนสิงหาคม พ.ศ.2556 แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 26 รายจับคู่กลุ่มทดลองและกลุ่มควบคุมให้มีลักษณะคล้ายคลึงกันในเรื่องระดับการศึกษา กลุ่มทดลองได้รับการสอนด้วยเทคนิคไม่ใช้มือจับต้อง ส่วนกลุ่มควบคุมได้รับการดูแลตามปกติ เครื่องมือที่ใช้ดำเนินการวิจัย ได้แก่ แผนการสอนโดยใช้เทคนิคไม่ใช้มือจับต้องแบบประเมินเทคนิคการให้นมมารดา และคู่มือการจัดท่าให้นมและการอมหัวนมของบุตร เครื่องมือรวบรวมข้อมูล ได้แก่ แบบสังเกตปัญหาในการเลี้ยงบุตรด้วยนมมารดาก่อนจำหน่ายจากโรงพยาบาล แบบสอบถามปัญหาในการเลี้ยงบุตรด้วยนมมารดาที่ 6 สัปดาห์หลังคลอด และแบบสอบถามความสำเร็จในการเลี้ยงบุตรด้วยนมมารดาอย่างเดียวของโบว์ชมพู บุตรแสงดี, กรรณิการ์ กันธะรักษา, จันทรรัตน์ เจริญสันติ (2556) วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนาและทดสอบสมมติฐานด้วยสถิติทดสอบไคสแควร์

Exclusive breastfeeding has enormous benefits for mothers and infants, including the impact on the family, society and nation. One important factor that can enhance the success of exclusive breastfeeding is mother’s breastfeeding technique. The purposeof this quasi-experimental research study was to study the effect of teaching using a hands off technique on successful exclusive breastfeeding among primiparous mothers. The sample consisted of 52 first-time mothers who were admitted to the postpartum care unit and had postpartum check-up at Tertiary Hospital, Chiang Mai, from May to August 2013. Subjects were assigned into either experimental and control group with twenty-six in each group. Each pair of subjects was matched based on education levels. The experimental group received the hands off technique instruction and the control group received conventional nursing care. The instruments for the intervention consistedof the hands off technique in breastfeeding plan, the mother’s breastfeeding technique assessment form and the manual of breastfeeding position and attachment. Instrumentsfor data collection were the observational form for breastfeeding problems beforedischarge, the 6th week postpartum breastfeeding problems questionnaire and the successful exclusive breastfeeding questionnaire by Bowchompoo Butsangdee (2013). Data were analyzed using descriptive statistics and Chi-square test.
