การศึกษาข้อมูลผู้ป่วยที่เข้ารับการรักษาทางศัลยกรรมจัดกระดูกขากรรไกรย้อนหลัง 16 ปี ของคณะทันตแพทยศาสตร์ มหาวิทยาลัยเชียงใหม่

Abstract

สภาพวิรูปของกระดูกขากรรไกรและใบหน้าเป็นความ ผิดปกติที่พบได้บ่อยซึ่งส่งผลต่อการทำหน้าที่ และความ สวยงามของใบหน้า โดยในปัจจุบันพบมีผู้ป่วยจำนวนมาก ขึ้นเป็นลำดับที่เข้ารับการรักษาทางศัลยกรรมจัดกระดูกขา กรรไกรที่คณะทันตแพทยศาสตร์ มหาวิทยาลัยเชียงใหม่ การศึกษาครั้งนี้จึงมีวัตถุประสงค์เพื่อศึกษาย้อนหลังและ เก็บรวบรวมข้อมูลทางสถิติของผู้ป่วยที่ได้รับการรักษาทาง ศัลยกรรมจัดกระดูกขากรรไกร ของคณะทันตแพทยศาสตร์ มหาวิทยาลัยเชียงใหม่ ในช่วงเวลา 16 ปีที่ผ่านมา เพื่อ ประโยชน์ต่อทีมทันตแพทย์และเจ้าหน้าที่ในการพัฒนาการ ให้บริการแก่ผู้ป่วย การศึกษาครั้งนี้ได้ทำการเก็บรวบรวม ข้อมูลย้อนหลัง ตั้งแต่เดือนมกราคม พ.ศ. 2539 ถึง เดือน ธันวาคม พ.ศ. 2555 จากข้อมูลพบมีจำนวนผู้ป่วยทั้งสิ้น 222 ราย เป็นผู้ป่วยหญิง 142 รายและผู้ป่วยชาย 80 ราย มีอายุเฉลี่ย 22.8 ปี แบ่งประเภทผู้ป่วยตามการวินิจฉัย พบ ความผิดปกติของความสัมพันธ์ของกระดูกขากรรไกรชนิดที่ สามมีจำนวนมากที่สุด การผ่าตัดมีหลากหลายเทคนิคแต่ เทคนิคที่ใช้บ่อย ได้แก่ การตัดกระดูกแบบเลอฟอร์ทวัน และบีเอสเอสอาร์โอ ระยะเวลาผ่าตัดเฉลี่ยของการผ่าตัด ทั้งหมด คือ 3 ชั่วโมง 20 นาที ระยะเวลานอนพักฟื้นเฉลี่ย ของผู้ป่วยทุกรายนาน 3.1 วัน และความรู้สึกสัมผัสเพี้ยนที่ ริมฝีปากล่างเป็นภาวะแทรกซ้อนหลังผ่าตัดที่พบมากที่สุด

Dento-facial deformity is one of the common disorders that has been frequently found. It negatively impacts on the function and the aesthetics of a patient’s face. Nowadays, there are a large number of patients who have undergone the orthognathic surgery at Faculty of Dentistry, Chiang Mai University. This research, therefore, is a retrospective study that aims to collect and review the statistical analysis of patients who have undergone the orthognathic surgery at Faculty of Dentistry, Chiang Mai University during the last 16 years. It is also helpful for dentists and officers to improve patient service. The statistics, which is the time between January 1996 and December 2012, revealed that there were 222 patients who had undergone the orthognathic surgery within this period of time; 142 of the patients were female whereas 80 patients were male. Besides, the patients’ average age was 22.8 years. The patients were divided based on the types of diagnosis which found the most deformity was skeletal class III . The surgical techniques were varies but the most surgical technique was Lefort I osteotomy and bilateral sagital split ramus osteotomy. The average duration of all surgeries was 3 hours 20 minutes. Moreover, the average number of patients’recovery period was 3.1 days. The Paresthesia of lower lip was the complication that had been mostly found.
