Abstract: This study aims to investigate the ramifications of transferring the mission of health-promoting sub-district hospitals to the provincial administrative organization on service recipients in Amphoe Mueang, Phayao Province. Using a mixed methods research approach, the objectives include elucidating the impact on service access, analyzing the community's healthcare service accessibility, and presenting significant challenges and obstacles in healthcare provision under these circumstances. The study sampled 10 key stakeholders, comprising administrators, public health department directors, and service recipients from health-promoting sub-district hospitals in Amphoe Mueang, totaling 397 individuals. The findings reveal a significant impact on healthcare service recipients, with an overall high level of impact associated with public healthcare service access, averaging at 4.92. Key aspects of service accessibility include service recipients' access to healthcare services in the Amphoe Mueang area, which was rated as high (average score of 4.92). The statistics from the study revealed that the overall level of access to public health services among the population in Mueang District, Phayao Province, was high (mean = 4.92). When considering each aspect, it was found that the aspects with the highest means were the acceptance of service quality (mean = 4.94) and service recipient satisfaction (mean = 4.94), followed by rehabilitation, affordability, and convenience and facilities, all with the same mean of 4.92. The aspect of treatment had the second-lowest mean at 4.88, while the aspect with the lowest mean was access to medical supplies at 4.86. These findings indicate that the population in Mueang District, Phayao Province, has good access to public health services, particularly in terms of the accepted quality of services and the satisfaction of service recipients. However, there are some aspects that may require further improvement, such as access to medical supplies, to further enhance the efficiency of public health service access for the population. Several critical challenges and obstacles were identified in healthcare provision within health-promoting sub-district hospitals in Amphoe Mueang, Phayao Province. These include shortages of medical personnel, insufficient equipment and amenities, difficulties in accessing services, which hinder individuals from accessing necessary healthcare, and issues in management and administration. This study contributes to the understanding of the repercussions of hospital mission transfers on healthcare service provision in rural areas. The findings underscore the importance of addressing these challenges to ensure equitable and accessible healthcare services for all. Further research is warranted to explore potential solutions and interventions to mitigate these obstacles and enhance healthcare delivery effectiveness in similar contexts.
