dc.description.abstract	This article attempts to use the location-allocation problem to decide the location of the pharmacy as a hub for dispensing medication instead of directly postal sending to the patient in the urban area of Chiang Mai, Thailand, to improve the quality of service and reduce the shipping cost of the hospital. The binary linear programming mathematical model has been conducted to calculate the proper location of the pharmacy to minimize the patient’s travel distance. The mathematical model has been solved using OpenSolver software, then calculate the shipping cost by the selected pharmacy that retrieved from the mathematical model result. The result shows that the hospital can reduce the shipping cost by up to 84.18%, but some patient might have to travel up to 6.27 kilometers to receive their medicine. On the other hand, if the maximum patient travel distance has been set to not more than 3 kilometers, the shipping cost reduction will subside to 50.50%. In addition, the sensitivity analysis shows that even if the shipping cost per unit has been raised to 2.4 times over the current rate, dispensing medicine via pharmacy will still reduce the shipping cost.	en_US

thesis.description.thaiAbstract	งานวิจัยฉบับนี้มุ่งเน้นการแก้ไขปัญหาเพื่อเลือกตำแหน่งที่ตั้งของร้านยาที่เหมาะสมสำหรับเป็นจุดให้บริการจ่ายยาทดแทนการจัดส่งยาไปยังที่อยู่ของผู้ป่วยโดยตรงของโรงพยาบาลมหาราชนครเชียงใหม่สำหรับผู้ป่วยที่อาศัยในเขตพื้นที่อำเภอเมืองเชียงใหม่และอำเภอข้างเคียง โดยมีวัตถุประสงค์เพื่อเลือกตำแหน่งที่ตั้งของร้านยาที่เหมาะสมสำหรับการเป็นจุดให้บริการจ่ายยา และเพื่อลดต้นทุนจากการเปลี่ยนแปลงรูปแบบการให้บริการ โดยในงานวิจัยฉบับนี้ได้ดำเนินการเก็บข้อมูลและสร้างตัวแบบทางคณิตศาสตร์ชนิดกำหนดการเชิงเส้นทวิภาคสำหรับเลือกตำแหน่งที่ตั้งของร้านยาและจัดสรรการเข้ารับบริการให้กับผู้ป่วยแต่ละรายเพื่อให้ผู้ป่วยสามารถเดินทางไปรับยาที่ร้านยาโดยมีระยะทางรวมสั้นที่สุด และทำการหาคำตอบผ่านโปรแกรมสำเร็จรูป OpenSolver หลังจากนั้นได้ทำการคำนวณต้นทุนที่เปลี่ยนแปลงไปจากการเปลี่ยนรูปแบบการให้บริการ โดยผลการศึกษาพบว่าหากเปลี่ยนรูปแบบการให้บริการจากการส่งยาไปยังที่อยู่ของผู้ป่วยโดยตรงเป็นการให้บริการจ่ายยาผ่านร้านยาที่อยู่ในพื้นที่ของผู้ป่วย โรงพยาบาลจะสามารถลดต้นทุนในการจัดส่งยาได้สูงสุดถึงร้อยละ 84.18 แต่ผู้ป่วยอาจต้องเดินทางมารับยาที่ร้านยาไกลถึง 6.27 กิโลเมตร แต่ในขณะเดียวกันหากกำหนดให้ผู้ป่วยเดินทางมารับยาที่ร้านยาในระยะทางไม่เกิน 3 กิโลเมตร จะต้องใช้ร้านยาเป็นจุดให้บริการสูงถึง 59 แห่ง และจะลดต้นทุนการจัดส่งได้เพียงร้อยละ 50.50 นอกจากนั้นผลการวิเคราะห์ความไวยังพบว่าการส่งไปยังร้านยาแทนการส่งยาไปยังที่อยู่ของผู้ป่วยโดยตรงยังสามารถลดต้นทุนการจัดส่งโดยรวมได้ถึงแม้ต้นทุนการจัดส่งต่อครั้งจะเพิ่มขึ้นสูงถึง 2.4 เท่าจากอัตราค่าขนส่งในปัจจุบันก็ตาม	en_US
