การวิเคราะห์สถานการณ์การนิเทศทางการพยาบาลของผู้ตรวจการพยาบาล นอกเวลาราชการห้องผ่าตัด โรงพยาบาลพุทธชินราช พิษณุโลก

Abstract

การนิเทศทางการพยาบาลเป็นเครื่องมือในการพัฒนาคุณภาพการบริการในโรงพยาบาลและเป็นกิจกรรมสำาคัญของผู้บริหารทุกระดับ การศึกษาเชิงพรรณนานี้มีวัตถุประสงค์เพื่อวิเคราะห์สถานการณ์การนิเทศทางการพยาบาลของผู้ตรวจการพยาบาลนอกเวลาราชการและเพื่อเสนอแนะแนวทางการแก้ปัญหาการนิเทศทางการพยาบาลของผู้ตรวจการพยาบาลนอกเวลาราชการ ห้องผ่าตัด โรงพยาบาลพุทธชินราช พิษณุโลก โดยใช้แนวคิดการประเมินคุณภาพของ Donabedian (2003) ประกอบด้วย 3 ด้าน คือ ด้านโครงสร้าง ด้านกระบวนการ และด้านผลลัพธ์กลุ่มประชากรทั้งหมดจำานวน 35 คน ประกอบด้วย ผู้ตรวจการพยาบาลนอกเวลาราชการ จำานวน 5 คน และพยาบาลวิชาชีพเวรนอกเวลาราชการ จำานวน 30 คน เครื่องมือที่ใช้ในการศึกษาประกอบด้วย แนวคำาถามสำาหรับการสัมภาษณ์รายบุคคลและแนวคำาถามสำาหรับการประชุมกลุ่ม วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนาและการวิเคราะห์เนื้อหา ผลการศึกษาพบว่า 1. ด้านโครงสร้าง พบว่า ห้องผ่าตัดมีการกำาหนดนโยบายและรูปแบบการนิเทศทางการพยาบาลของผู้ตรวจการนอกเวลาราชการยังไม่ชัดเจน มีการวางแผนการนิเทศในแต่ละเวร แต่ไม่มีการจัดทำารูปเล่มเป็นลายลักษณ์อักษรแบบพรรณนาลักษณะงานของผู้ตรวจการไม่มีความเฉพาะเจาะจง และเครื่องมือที่ใช้ในการนิเทศทางการพยาบาลของผู้ตรวจการมีน้อยและไม่ครอบคลุม จึงเสนอแนะให้มีการจัดทำานโยบายและรูปแบบการนิเทศทางการพยาบาลเพื่อให้ผู้ตรวจการนอกเวลาราชการปฏิบัติงานตามมาตรฐานงานห้องผ่าตัด จัดทำาแบบฟอร์มการนิเทศของผู้ตรวจการจัดทำาพรรณนาลักษณะงานให้ชัดเจนและมีแนวทางปฏิบัติในการใช้เครื่องมือการนิเทศที่เป็นลายลักษณ์อักษรชัดเจนสามารถปฏิบัติงานไปในแนวทางเดียวกัน 2. ด้านกระบวนการ พบว่า การวางแผนในการนิเทศไม่ชัดเจนและไม่เป็นรูปธรรม การกำาหนดโครงสร้างใน การนิเทศไม่ชัดเจน การสร้างแรงจูงใจให้ผู้รับการนิเทศไม่เพียงพอ การควบคุมการปฏิบัติงานมีรูปแบบไม่ชัดเจน และไม่มีแบบประเมินที่เป็นรูปธรรม ดังนั้นเสนอแนะให้มีการกำาหนดบทบาทหน้าที่ของผู้ตรวจการในแต่ละวัน มีการจัดโครงสร้างการปฏิบัติงานให้รู้บทบาทและหน้าที่ของการนิเทศ ควรมีการจัดทำาแบบฟอร์มที่ใช้ในการควบคุมการปฏิบัติงาน และจัดทำาแบบประเมินการปฏิบัติงาน 3. ด้านผลลัพธ์ พบว่า ผู้รับการนิเทศมีความพึงพอใจบางส่วนในการปฏิบัติงานของผู้ตรวจการนอกเวลาราชการเสนอแนะให้ส่งเสริมผู้ตรวจการได้รับการอบรมเกี่ยวกับการนิเทศทางการพยาบาล ดังนั้นจึงควรมีการนำาเสนอผลการศึกษาต่อผู้บริหารทางการพยาบาลเพื่อนำาไปพัฒนารูปแบบการนิเทศทาง การพยาบาลของผู้ตรวจการพยาบาลนอกเวลาราชการ

Nursing supervision is a tool to improve the quality of services in hospitals and the main activity of executives at all levels. This descriptive study aimed to analyze the situation of nursing supervision among unofficial-time nurse supervisors and offer the solutions of unofficial-time nursing supervision in the Operating Room, at Buddhachinaraj Phitsanulok Hospital. The Donabedian quality assessment (2003) was used as the conceptual framework for this study. Consisting of 3 aspects which are structure, process, and outcomes. The total population is 35 people consisting of 5 unofficial-time nurse supervisors and 30 unofficial-time nursing personnel. The research instruments were the questions for individual interviews and questions for the group meeting. Datawere analyzed using descriptive statistics and content analysis.The results revealed that: 1. For structure, the operating room had a policy and model of nursing supervision among unofficial-time nurse supervisors but not clear. There was a supervision plan in each shift but no preparation. The job description of nursing supervision among unofficial-time nurse supervisors was unspecific. A few instruments used in nursing supervision were not covered. Suggestions to improve include preparing a policy for nursing supervision and establishing a model of nursing supervision based on the guidelines of the operating room, creating a nursing supervision form, preparing a clear job description, and conducting concrete practice guidelines for using supervision instruments-so that work can be done in the same way. 2. Regarding process, the nursing supervision plan was not clear and concrete. The supervision structure was not clear. Facilitating motivation was inadequate, the controlling has unclear modeland there was a lack of concrete evaluation. Therefore, included stating the roles and responsibilities for daily work, establishing a supervision structure for roles and responsibilities for supervision, preparing evaluation forms used in the monitoring process, and creating a work performanceevaluation process. 3. Regarding the outcomes, it was found that supervision recipients were somewhat satisfied with the performance of nurse supervisors. Suggestions to improve include encouraging nurse supervisors to be trained about nursing supervision. The results of this study could be proposed to nursing administrators in order to improve the nursing supervision model of unofficial-time nurse supervisors.
