บทบาทพยาบาลวิชาชีพในการพัฒนานวัตกรรมการสร้างเสริมสุขภาพภาคเหนือ

Abstract

การบูรณาการการสร้างเสริมสุขภาพของประเทศไทยในบริการสุขภาพยังไม่ชัดเจนมากนัก จากการสำรวจ สัมภาษณ์ และสนทนากลุ่มด้านนวัตกรรมการสร้างเสริมสุขภาพ ของสถานบริการสุขภาพทั้งระดับปฐมภูมิ ทุติยภูมิ และตติยภูมิในเขตภาคเหนือ รวมทั้งใช้เกณฑ์การคัดเลือกที่ตั้งขึ้น พบว่ามีนวัตกรรมการสร้างเสริมสุขภาพ 5 โครงการ ได้แก่ 1) รูปแบบการดำเนินงานการดูแลผู้สูงอายุระยะยาว โรงพยาบาลส่งเสริมสุขภาพตำบล ท่ากว้าง อำเภอสารภี จังหวัดเชียงใหม่ 2) การจัดการที่ดินโดยองค์กรชุมชน ตำบลแม่ทา อำเภอแม่ออน จังหวัด เชียงใหม่ 3) แม่ลาวโมเดล “สุขภาพดีวิถีธรรมชาติ” โรงพยาบาลแม่ลาว อำเภอแม่ลาว จังหวัดเชียงราย 4) การ ดูแลแผล Ileostomy แบบบูรณาการ โรงพยาบาลสันป่าตอง จังหวัดเชียงใหม่ 5) การพัฒนาเชิงระบบในการ ป้องกันดูแลแผลกดทับ โรงพยาบาลมหาราชนครเชียงใหม่ จังหวัดเชียงใหม่ บทบาทของพยาบาลในการ สร้างเสริมสุขภาพ พบว่าครอบคลุมทั้ง 5 ยุทธศาสตร์ออตตาวา ได้แก่ 1) การสร้างนโยบายสาธารณะเพื่อสุขภาพ 2) การสร้างสิ่งแวดล้อมที่เอื้อต่อการมีสุขภาพดี 3) การสร้างชุมชนเข้มแข็ง4) การพัฒนาทักษะส่วนบุคคล และ 5) การปรับระบบบริการสุขภาพ อย่างไรก็ตาม ลักษณะการดำ เนินงานในแต่ละบทบาท จะมีจุดเด่นในการ สร้างเสริมสุขภาพที่แตกต่างกันทั้งนี้ขึ้นอยู่กับประเด็นในการสร้างเสริมสุขภาพของแต่ละบริบทที่แตกต่างกัน จากการวิเคราะห์นวัตกรรมทั้งห้า พบว่าบทบาทของพยาบาลที่ได้ปฏิบัติมากที่สุด คือ การพัฒนาทักษะ ส่วนบุคคล แต่สิ่งที่พยาบาลได้ปฏิบัติน้อยที่สุด คือ การสร้างนโยบายสาธารณะเพื่อสุขภาพ ผลการวิเคราะห์นี้ ควรได้รับการเผยแพร่ ให้ทราบโดยทั่วกัน เพื่อสามารถนำนวัตกรรมไปประยุกต์ใช้ และควรจัดทำฐานข้อมูลนวัตกรรมการสร้างเสริมสุขภาพ และควรสนับสนุนให้เกิดนวัตกรรมการสร้างเสริมสุขภาพเพื่อสุขภาวะของ ผู้ใช้บริการและผู้ให้บริการสุขภาพ

The integration of health promotion into nursing services is not yet clear. In this study, surveys, interviews, and focus group discussions of health promotion innovation in the primary, secondary, and tertiary health care levels in the Northern region of Thailand were used to investigate this integration. Using the developed selection criteria, five health promotion innovations were selected, which were 1) Model Long Term Care, Tha Kwang Health Promoting Hospital, Sarapee District, Chiang Mai, 2) Land Management by Local Organizations, Tambon Mae Tha, Mae On District, Chiang Mai, 3) “Healthy Pathway by Nature", Mae Lao Hospital, Mae Lao District, Chiang Rai, 4) Ileostomy Integrated Wound Care, Sanpatong Hospital, Chiang Mai, and 5) Systematic Development Program for Pressure Sore Care, Maharaj Nakorn Chiang Mai Hospital, Chiang Mai. In these innovation models, nurses’ roles covered all five strategies based on the Ottawa Charter including building healthy public policy, strengthening community action, developing personal skills, and reorienting health. However, nurses’ roles in implementing each health promotion innovation differed according to the differences of each context. From an analysis of these five health promotion innovation projects, the most evident nurse's role was the development of personal skills and the least evident role was the building of healthy public policy. This analysis should be publicized so it can be used. Furthermore, a health promotion innovation database should be established and health promotion innovation should be promoted due to its significance regarding health care provider's services and the health of clients.
