ผลของการสนับสนุนทางสังคมต่อความสำเร็จในการเลี้ยงบุตรด้วยนมมารดาอย่างเดียวของมารดาวัยรุ่น

Abstract

มารดาวัยรุ่นส่วนมากมักขาดความรู้ ทักษะ และความมั่นใจในการเลี้ยงบุตรด้วยนมมารดา ซึ่งมีผลต่อความสำเร็จในการเลี้ยงบุตรด้วยนมมารดาอย่างเดียว พยาบาลผดุงครรภ์สามารถส่งเสริมการเลี้ยงบุตรด้วยนมมารดาอย่างเดียวโดยให้การสนับสนุนมารดาวัยรุ่น การศึกษาวิจัยแบบกึ่งทดลองครั้งนี้มีวัตถุประสงค์เพื่อศึกษาผลของการสนับสนุนทางสังคมต่อความสำเร็จในการเลี้ยงบุตรด้วยนมมารดาอย่างเดียวของมารดาวัยรุ่น กลุ่มตัวอย่างเป็นมารดาวัยรุ่นจำนวน 52 ราย ที่มาฝากครรภ์ คลอด และพักรักษาตัวในแผนกหลังคลอด โรงพยาบาลกำแพงเพชร ระหว่างเดือนมิถุนายน พ.ศ. 2555 ถึงเดือนมกราคม พ.ศ.2556 คัดเลือกกลุ่มตัวอย่างตามคุณสมบัติที่กำหนด แบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 26คน โดยการจับคู่ให้มีลักษณะคล้ายคลึงกัน เครื่องมือที่ใช้ในการวิจัยครั้งนี้ คือ แผนการสนับสนุนทางสังคมเพื่อส่งเสริมความสำเร็จในการเลี้ยงบุตรด้วยนมมารดาอย่างเดียวของมารดาวัยรุ่น และคู่มือการเลี้ยงบุตรด้วยนมมารดาสำหรับมารดาวัยรุ่น เครื่องมือที่ใช้ในการรวบรวมข้อมูล คือ แบบสอบถามการเลี้ยงบุตรด้วยนมมารดาอย่างเดียวของโบว์ชมพู บุตรแสงดี(2554) วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา และการทดสอบไคสแควร์ ผลการวิจัยพบว่า มารดาวัยรุ่นในกลุ่มทดลองมีอัตราการเลี้ยงบุตรด้วยนมมารดาอย่างเดียวทั้งก่อนจำหน่ายจากโรงพยาบาล และที่ 6 สัปดาห์หลังคลอดมากกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p<0.001) ผลการวิจัยครั้งนี้ ช่วยให้พยาบาลผดุงครรภ์มีแนวทางในการส่งเสริมการเลี้ยงบุตรด้วยนมมารดาอย่างเดียวในมารดาวัยรุ่น และคาดว่าจะเป็นประโยชน์ต่อการปฏิบัติการพยาบาล การศึกษา และการวิจัยต่อไป

Most adolescent mothers lack breastfeeding knowledge, skills, and confidence whichgreatly influences their exclusive breastfeeding success. Nurse-midwives can promote exclusive breastfeeding through support adolescent mothers. The purpose of this quasi-experimental research was to examine the effect of social support on exclusive breastfeeding success among adolescent mothers. The subjects consisted of 52 adolescentmothers who attended the antenatal clinic and were admitted to the labor and postpartum ward at Kamphaengphet Hospital from June 2012 to January 2013. They were selected using the inclusion criteria and were equally assigned into the control or experimental group, with twenty-six mothers in each group. Each subject pair was matched. The researchinstruments were thesocial support plan for promoting success in exclusive breastfeeding among adolescent mothers and breastfeeding manual for adolescent mothers. Instrument for data collection was an exclusive breastfeeding questionnaire byBudsaengdee (2011). Data were analyzed using descriptive statistics and Chi-square test. The finding of this studyrevealed that adolescent mothers in the experimental group had significantly higher exclusive breastfeeding rates both before discharge from hospital and at six weeks postpartum than that of the control group (p<0.001). The study results provide nurse-midwives with guidance for promoting exclusive breastfeeding among adolescent mothers. The finding of this study will be valuable for nursing practice, education, and research.
