การพัฒนาคุณภาพการบันทึกทางการพยาบาลในโรงพยาบาลนครพิงค์ จังหวัดเชียงใหม่

Abstract

บันทึกทางการพยาบาลเป็นหลักฐานสำคัญในการสะท้อนการพยาบาลที่ผู้ป่วยได้รับ และสามารถใช้ในการสื่อสารระหว่างบุคลากรพยาบาล ดังนั้นการบันทึกทางการพยาบาลที่มีคุณภาพจึงมีความสำคัญต่อการปฏิบัติการพยาบาล การศึกษาเชิงพัฒนานี้มีจุดมุ่งหมายเพื่อพัฒนาคุณภาพการบันทึกทางการพยาบาลในหอผู้ป่วยออร์โธปิดิกส์ โรงพยาบาลนครพิงค์ จังหวัดเชียงใหม่ วิธีดำเนินการศึกษาอาศัยกระบวนการปรับปรุงคุณภาพโฟกัสพีดีซีเอ (Deming, 1993 as sited in McLaughlin & Kaluzny, 1999) ซึ่งประกอบด้วย 9 ขั้นตอน ได้แก่ ค้นหากระบวนการในการปรับปรุงคุณภาพ สร้างทีมงานให้มีความรู้ความเข้าใจเกี่ยวกับกระบวนการ ทำความเข้าใจกระบวนการที่จะปรับปรุง ทำความเข้าใจถึงสาเหตุของความแปรปรวนของกระบวนการ เลือกวิธีการปรับปรุงกระบวนการ วางแผนในการปรับปรุง นำไปปฏิบัติ ตรวจสอบการปฏิบัติและยืนยันการดำเนินการและปรับปรุงอย่างต่อเนื่อง ประชากรในการศึกษาครั้งนี้คือ พยาบาลวิชาชีพจำนวน 11 คน เครื่องมือที่ใช้ในการดำเนินการศึกษาได้แก่ แนวคำถามในการประชุมกลุ่ม และแบบประเมินคุณภาพการบันทึกทางการพยาบาลในหอผู้ป่วยออร์โธปิดิกส์ที่ผู้ศึกษาสร้างขึ้น วิเคราะห์ข้อมูลด้วยสถิติบรรยาย ผลการศึกษาพบว่า กระบวนการโฟกัสพีดีซีเอช่วยในการปรับปรุงการบันทึกทางการพยาบาล ในการศึกษานี้ได้ โดยช่วยให้พยาบาลสามารถระบุสาเหตุของการบันทึกทางการพยาบาลที่ไม่สมบูรณ์ และเลือกแนวทางที่เหมาะสมในการแก้ไขปัญหา ผู้วิจัยได้มีการพัฒนาแนวทางในการเขียนบันทึกทางการพยาบาลและอบรมให้พยาบาลปฏิบัติตามแนวปฏิบัติหลังจากดำเนินการ 1 เดือน ผู้วิจัยได้ตรวจสอบบันทึกทางการพยาบาล และพบว่าคุณภาพการบันทึกทางการพยาบาลเพิ่มขึ้นจากเดิมร้อยละ 41.64 เป็นร้อยละ 83.70 ผลการศึกษานี้แสดงว่า กระบวนการโฟกัสพีดีซีเอสามารถนำไปใช้ได้จริงในการพัฒนาคุณภาพการบันทึกทางการพยาบาล ซึ่งผู้บริหารองค์การสามารถนำไปใช้ในการพัฒนาคุณภาพการบริการในหน่วยงานตนเองหรือหน่วยงานอื่น ๆ ในองค์การได้

Nursing document serves as important evidence that shows nursing care provided to patients; it can also be used as a form of communication among nursing personnel. Effective nursing documentation is therefore crucial for nursing practice. The purpose of this developmental study was to improve the quality of nursing documentation in Orthopedics Ward Nakornping Hospital. Study methodology was guided by the FOCUS PDCA improvementprocess (Deming, 1993 as sited in McLaughlin & Kaluzny, 1999) which consists of nine steps: find a process to improve; organize a team that knows the process; clarify current knowledge of the process; understand causes of process variation; select the processes improvement; plan the improvement; do the improvement; check the result, data collection and analysis; and act to hold the gain and continue improvement. The population in this study was 11 registered nurses. The research instruments included an interview guide and assessment form for nursing documentation which were developed by the researcher. Data were analyzedusing descriptive statisticsResults revealed that FOCUS PDCA methodology were able to improve nursingdocumentation in this study. The process helped nurses identify the causes of uncompleted nursing documents and select appropriate actions to solve identified problems. The researcherdeveloped practice guidelines for nursing documentation and trained nurses for practice compliance with the guidelines. One month after implementation, a chart audit was done by the researcher and the quality of nursing documentation increased from 41.64% to 83.70%. The results of the study show that FOCUS PDCA methodology helps to improve the quality of nursing documentation. Administrators can apply it to improve quality of services in their departments and other departments in the organization.
