Abstract: Hypertension is the most common chronic disease among the elderly which can result in dangerous complications if their blood pressure is not controlled. Self-care ability is an important factor in controlling blood pressure. This operational study aimed to evaluate the effectiveness of clinical practice guidelines implementation for promoting self-care ability among elderly with hypertension who received services at Maetha Hospital in Lampang province between May to August 2014. The subjects consisted of 3 registered nurses and 60 elderly with hypertension who had low or lowest levels of self-care ability scores. The research instrument was the Clinical Practice Guidelines (CPGs) for Promoting Self-Care Ability Among Elderly with Hypertension, developed by Somrat (2008). The effectiveness of Clinical Practice Guidelines implementation was evaluated based on clinical outcomes including self-care ability, satisfaction with care received according to the guidelines, and satisfaction of nurses. Data were analyzed using descriptive statistics. The results of this study revealed that: 1. Patients receiving usual care received low and lowest levels of self-care ability scores, 0.17 and 0.83 respectively. Comparatively, patients receiving care based on the CPGs received high and highest levels of self-care ability scores, 0.33 and 0.67, respectively. 2.All nurses had high levels of satisfaction with the CPGs. 3. All patients had high levels of satisfaction with the service received based on the CPGs. Study results indicate that the clinical practice guidelines was successful in promoting the self-care ability of the elderly with hypertension. Therefore, registered nurses are encouraged to use these CPGs to improve the quality of care of the elderly with hypertension who receive services at Maetha Hospital.
